An Accountable Care Organization (ACO) is a way for medical providers to have greater insight into the care of their patients. In fact we now share in the "risk" of your treatment. We want to keep you healthy and away from the emergency department! Some great tips for you:

• Take Medications as directed.

• Keep track of your numbers, BP, A1c and Weight.

• Keep your appointments and call us first when issues begin.
